Before sessions begin the relevant agreement must be signed. Please choose the appropriate one below and fill in the form.
- Child Coaching Agreement (U16s) to be signed by parent/carer
- Child Coaching Agreement where parents are separated
- 16-18 Year Old Coaching Agreement to be signed by both parent/carer & young person
- Adult Coaching agreement
- Benefactor/Organisation Coaching Agreement - to be signed by benefactor and coachee
- Child/Benefactor Coaching Agreement to be signed by parent/guardian
- Speech & Language Therapy Agreement
